Required Materials for Application Packet:

  1. Online Application Form
    1. Apply in English
    2. Solicitar en Español
  2. Records submitted via email or sent directly the school
    1. Copy of Birth certificate
    2. Copy of Baptismal record (if baptized)
    3. Report cards or transcripts from outside academic institutions
  3. Letter of Recommendation and evaluation of child’s performance (submitted by teacher and/or administrator from their previous academic institution) submitted in via email or sent directly to the school
  4. Letter of Recommendation and Evaluation Waiver Form (submitted by the parent/guardian) to the school via email or sent directly to the school
